Intake Process Manual
October 2015 - July 2017
Consolidated and improved the referral intake process for a nonprofit hospice. Worked with all levels of personnel from frontline operators to the vice president and across multiple departments from Compliance to IT to create a new Intake Process Manual. The project reduced the time to upload medical history to the computer system from days to hours and drastically simplified the overall intake process from initial phone call to the scheduling of appointments and admission to services.

Discovering CHOICE Project Management and Evaluation
June 2015 - September 2017
Provided project management and evaluation services for three nonprofits as they implemented a new program across five locations for adults with developmental disabilities. Responsibilities required coordination of meetings, logistics, reporting, and communications between three separate agencies encompassing five sites and over 20 individuals.
Discovering CHOICE Manual
September 2016 - April 2017
Managed project to create and publish a Program Manual for a new type of day program for adults with intellectual and developmental disabilities (I/DD) for distribution to similar agencies across the country. Responsibilities included drafting the outline, working with executives to refine content, coordinating with the graphic designer on layout and design, and writing the text of the manual.

CHOICE Evaluation
November 2013 - March 2015
Developed an evaluation plan for a new day program for adults with developmental disabilities using a participatory process with several staff members. The evaluation plan outlines seven key areas to monitor during the first year of the program including facilities, advisory committee, curriculum, staff, program outreach, iPad usage, and consumer satisfaction. Project tasks include facilitating a quarterly meeting with the advisory committee and staff and generating quarterly reports.

IPU Volunteer Management Project
January 2014 - October 2014
Led a group of seven volunteers and employees in a six-month initiative to reignite the In-Patient Unit (IPU) Volunteer system for a local hospice. Responsibilities included defining new volunteer roles, creating job descriptions and procedures, and developing training materials. The project contributed to an increase of 167% in the number of volunteer shifts between January 2014 and July 2014.

Internal Be@Ease Initiative
February 2013 - September 2013
Developed and managed the first phase of the interal JourneyCare Be@Ease project focusing on increasing staff completion of advance care planning documents. Developed logic model and project plan. Created, administered, and analyzed pre-project survey. Designed intranet site and comprehensive guide to advance care planning. First phase activities resulted in 85% staff participation.
Consumer Satisfaction Survey
May 2012 - May 2013
Created online client and family satisfaction survey to assess services for 1000+ clients resulting in a 36% response rate. Consolidated eight existing paper surveys into one comprehensive online survey covering all adult services. Piloted survey, developed communications, coordinated survey distribution, designed and delivered training to staff on how to analyze and report survey results.
